Please Help us to Help You

If you cannot keep an appointment, please let us know so that we can offer it to another patient.  Due to unfortunately high numbers of missed appointments it has been necessary to implement the following policy:

  • If you fail to attend for 3 appointments without letting us know prior to the appointment time, you may be removed from the Practice list and will have to find an alternative doctor.
  • If you change your name, please let us know so that we can update your records.
  • If you move, please let us know your new address.  If it is outside our Practice area we will need to transfer your records to your new Doctor.
  • If you change your telephone number, please let us know so that we can update your records.

What to do in an emergency

Whatever the day or time, if you or someone else experiences severe chest pain, loss of blood or suspected broken bones, go to your nearest accident and emergency department or call 999.


For the Doctor

The Surgery operates an appointment system.  Appointments for Doctors can be pre-booked up to three weeks in advance. For a more urgent appointment on the same day please ring the Surgery, these appointments are time controlled and are not released until 8.45am.

You will be given the first available appointment to see the Doctor of your choice.  Children under 16 must be accompanied by an adult.

You will be encouraged to see the Doctor that you have seen previously with the same complaint.

Patients are able to see a Health Professional of their choice.  If you do not express a preference you will be given an appointment in the next available clinic.

Appointments are usually for ten minutes unless it is for minor Surgery or a medical examination, which takes longer.  Please inform the receptionist if this is the case.

Please ask at Reception if you would like to register with the practice to book appointments on-line from  your home/work computer.


We aim to offer a personal and efficient service by offering both pre-bookable appointment sessions and “same day access” sessions.  It would be appreciated if ‘non working’ patients could leave early morning and late appointments for people at work who have difficulty coming during normal working hours.

Extended Access Service

West Lancashire now has an Extended Access Service running 7 days a week:

  • Monday – Friday 18:30-20:00
  • Saturdays – 10:00-16:00
  • Sundays – 10:00-14:00

The service runs from various locations on weeknights and from 3 hubs in West Lancs on the weekend:

  • Burscough Helth Centre
  • Ormskirk Medical Practice
  • Sandy Lane Health Centre

The service offers pre-bookable and same-day primary care appointments with a range of Clinicians including:

  • GPs
  • Nurses
  • Health Care Assistants

Appointments are available to all patients that are registered in West Lancs and can be booked by contacting reception.

Please be aware that this is not a walk-in service – you must have an appointment. Should you need to cancel your appointment, please contact the Extended Access Service directly on: 01695 736 053

Fit for Work Certificate

It is not necessary to have a Doctor’s certificate until you have been absent continuously from work for seven days including weekends. After three days you must obtain a self-certificate (SC2) from your employer. If your employer insists on a private certificate for an illness of less than seven days duration then the Doctor will charge a fee of £10. If your illness continues for more than seven days then under the NHS your GP can issue an official statement (Med3) “fit to work certificate” to confirm your illness.

Advice and Information by Telephone

There is a facility to make a 5 minute telephone consultation with a Doctor or Nurse at the end of morning surgery e.g.

  • Follow-up,
  • Medication review
  • Drug reaction
  • Test results etc.

You will be asked to provide a Telephone number, one telephone call will be made to this number; no message will be left on an answer machine, or to a third party.

Local Pharmacist

Your local pharmacist will be able to give you free health advice at any time – you do not need an appointment. Many pharmacies operate extended hours on a Rota basis. NHS Direct will have details.

Appointments for the Nurse

We operate a booked appointment system for all clinics. When booking your appointment please let Reception know what your appointment is for, so that they can advise you when would be the best time for you to come and the length of appointment you require.

Our District Nurse is available by appointment for:

  • blood tests
  • ear syringing
  • skin infections
  • wound dressings and minor injuries

She can also give advice on Asthma, Diet, Diabetes, Smoking Cessation and Weight Management.

Disabled Patients

Access ramps and disabled toilet facilities are available.  All consulting rooms are easily accessed

Home Visits

Please do not ask the Doctor to call unless the patient is genuinely too ill to come to Surgery.  If a patient has a temperature or a rash, coming to the Surgery will do no harm but please inform the Receptionist on arrival.  Please also remember the patient must agree to a home visit, without their consent the Doctor will not attend.


During Surgery hours an emergency is a medical problem that needs attention within 30 minutes. After hours it is a medical problem that cannot wait until the surgery opens.



Due to the ongoing situation with Coronavirus, we are taking measures in line with guidance shared by NHS England and Public Health Authorities to minimise risks associated with the virus.

In order to protect our patients and staff we are asking our patients to support us during this difficult time.

Ashurst Primary Care is open as usual to continue providing continuity of care to all of our patients.  Our main objective is to protect our patients and staff and therefore we will be limiting face to face contacts with effect from 17.03.2020. To that affect we are asking that you do not come into practice unless you are asked to by a GP or a staff member.

All patients who have a booked appointment will be contacted by a member of the team to convert your appointment to a telephone appointment.  Based on your telephone appointment the GP will decide if you need to be seen and arrange a time for you to attend the surgery.

From today all new requests to book an appointment will be telephone triaged by a Doctor.

We will not be accepting paper prescription requests at the practice, please order repeat prescriptions by ringing the POD on 01695 588522 or online using the MyGp app.  If you need online access setting up please contact the surgery by phone on 01695 736111.
